Compensation: $26 – $29/hour, commensurate with experience

NaviGuide San Diego (www.naviguidesandiego.org) is a new division of ElderHelp San Diego launched in partnership with United Church Homes of Ohio. United Church Homes is a national leader in senior housing and healthcare services with more than 100 years of experience.
NaviGuide San Diego is a non-profit caregiver program offering a subscription-based care management platform. We help older adults, and their caregivers manage the complexities of aging by providing personalized support, service coordination, and access to trusted resources.
Through in-person connections and ongoing relationships, NaviGuide Support Coordinators work closely with older adults and their families to develop tailored service plans that evolve as needs change. Whether arranging a little extra help at home or navigating more advanced care options, NaviGuide is built on a foundation of trust, compassion, and proactive support.

POSITION SUMMARY
The NaviGuide Support Coordinator provides direct services to older adults and their caregivers as well as supports the outreach and awareness of the program within San Diego County. The Coordinator is knowledgeable about community resources, is solution-oriented, and adept at helping clients navigate complex care systems. The Coordinator works collaboratively to coordinate programs and services that assist older adults and their caregivers in maintaining the highest practical level of independence and physical, mental, and psychological well-being while supporting caregiving decision-making and access to resources.
This is a hybrid position that involves approximately 80% driving within San Diego County in addition to office and administrative work, and telephone communications. Travel includes in-home assessments and client check-ins with the older adult community, visiting with community service providers and partners, conducting outreach at presentations, conferences, company events, and networking opportunities.
This position requires excellent written and oral communication skills and the ability to build strong relationships. As the role involves working with families in care navigation settings, the Coordinator must uphold high ethical standards and demonstrate sound judgement in all interactions. The Coordinator promotes client independence and wellness while supporting caregiving decisions with integrity, professionalism, and compassion.
ESSENTIAL DUTIES & RESPONSIBILITIES
Case Management
• Assesses client needs in person/in-home and over the phone to establish individual service plans
• Helps clients and their families acquire and utilize community services and supports needed to maintain an independent lifestyle
• Acts as a liaison between community agencies, service providers, clients, and their families; seeks out new services and identifies affordable providers
• Monitors the delivery of services to clients to ensure they are appropriate, timely, and satisfactory
• Maintains a resource directory of available community services and partners
• Provides regular communications with all clients and approved designated caregivers to identify areas of need and make referrals to community agencies as necessary
• Maintains client files, including but not limited to intake information, assessments, and service referrals
• Submits all required reporting data within specified timeframes
• Promotes wellness activities for clients, and encourages clients to be proactive in meeting their own social, psychological and physical needs
• Acts as a family support system and provides any needed community resources for caregivers
• Maintains accurate and timely notes on client, family, and provider meetings and communications
Outreach
• Networks with service providers to maintain necessary relationships and pursues additional services through private, local, state, and federal sources
• In partnership with the Business Development and Marketing Manager, assists with activities to market the program and provides educational presentations to build caseload.
General
• Advocates on clients’ behalf as needed
• Attends team meetings as directed
• Adheres to the NASW code of ethics
• Adheres to company policies and procedures
• Performs all other duties as assigned or directed
EDUCATION & QUALIFICATIONS
Education & Experience
• Bachelor’s Degree in Social Work or Gerontology preferred
• Three (3) or more years of experience working directly with seniors preferred
Qualifications
• Familiarity with local community resources and services
• Ability to analyze, monitor and evaluate programs and services
• Ability to advocate for and provide support on behalf of clients
• Ability to synthesize information from clients and community based on comprehensive, multidimensional assessments
• Knowledge of best practices in supporting client self-determination, goal setting, and access to community-based services
• Demonstrated ability to build strong relationships with internal teams and external partners, including clients, family members, legal representatives, healthcare entities, and housing staff
• Ability to help enhance clients’ motivation for change and cultivate hope while addressing ambivalence and barriers to change
• Detail oriented and goal driven
• Able to establish culturally competent relationships
• Self-motivated with the ability to take initiative to solve problems and reach goals
• Organized with strong follow-through and time management skills
• Able to work independently with flexibility, integrity, and sound judgement
• Ability to communicate clearly and effectively in English, both verbally and in writing
• Proficient with computers, Microsoft Office, the internet, and general office equipment
• Must have a reliable car and be able to travel throughout San Diego County
• Membership and familiarization with American Association of Service Coordinators helpful (will be provided by NaviGuide of San Diego)
PHYSICAL DEMANDS
This position requires regular standing, sitting, and use of standard office equipment. No lifting over 20 lbs is required.
