care manager provides information about
community resources including legal
help, health services, in home support,
transportation, food resources, housing,
and more.
 What is Information and Referral at ElderHelp? |
| Information and Referral is available at ElderHelp of San Diego on a call-in
or walk-in basis, Monday through Thursday, 8am to noon.

 Who
do we serve? |
| Seniors, family members, friends, neighbors, social workers, and all other members
of the community are free to utilize ElderHelp’s Information and Referral
services. Please keep in mind that ElderHelp serves seniors age 60 and
better, living in San Diego County, so the information available is geared specifically
toward this population.

 What
kinds of resources are provided? |
| Information regarding legal help, health services, transportation, food resources,
housing, in home support, utility discounts, adaptive equipment, and more is
all available from Information and Referral.
Additionally, Information
and Referral is the entry way
for requesting ElderHelp Care
Management and Volunteer Services. The
Information and Referral case
manager will assess one’s
eligibility for ElderHelp services,
do an initial intake, and refer
the individual on to the appropriate
ElderHelp program or service
that best fits their need.
