The Faith in Action Program
assists the frail and homebound elderly
or disabled who live alone, suffer chronic
health problems, or are unable to perform
simple daily tasks. Through a grant from
the nationally renowned Robert Wood Johnson
Foundation, Faith in Action enables ElderHelp
to create innovative partnerships with
faith-based organizations to offer services
to their elderly congregants. Seniors
referred to ElderHelp from Faith in Action
coalition members receive a home visit
from a care manager who assesses their
needs, generates referrals for volunteers
to meet those needs, connects with community
resources and provides follow-up to evaluate
progress.

Faith in Action serves
seniors such as 83-year-old
Zame, who has no family
in San Diego and is legally
blind. Prior to receiving
help from her ElderHelp
care manager, Beth, Zame
struggled taking the bus
to and from medical appointments
and to buy groceries.
Zame shared her struggles
with her pastor at Plymouth
Congregational Church,
a Faith in Action coalition
member, who referred her
to ElderHelp.
ElderHelp assigned Zame
a grocery shopping volunteer
and provides her with
taxi vouchers so she can
now get to her medical
appointments with ease.
In addition, a church
friend and newly recruited
Faith in Action volunteer
helps Zame sort her mail
and pay her bills.
Zame says, “I am
so grateful my church
told me about this program.
It means a great deal
to me. The public needs
to know about the dedicated
people at ElderHelp who
are concerned about the
elderly.”
