Care Management

Care managers develop individualized care plans and maintain regular contact with seniors to lengthen their time at home and avoid placement in nursing facilities.

Information & Referral

Information and Referral is available at ElderHelp of San Diego on a call-in or walk-in basis, Monday through Thursday, 8am to noon.

HomeShare

The HomeShare Program matches older homeowners who are looking for and could benefit from income, companionship and/or help in their homes, with people prepared to lend a hand in return for affordable accommodations.

Benefits Counseling

A listing of organizations that assist seniors in San Diego understand things such as legal issues, health insurance, and taxes.

LGBT Senior Health Project

Aging as Ourselves is designed to improve access by lesbian, gay, bisexual, transgender (LGBT) seniors to appropriate health care and social services. For more information, please see: AgingAsOurselves.org
