Our Care Coordination program provides comprehensive in-home solutions to address the growing health and social challenges that older adults and their families are facing. The program offers help with the basic necessities of life through personal attention, meaningful engagement and coordinated programs and services.
A Care Coordinator will help connect a variety of services using caring staff, dedicated volunteers and our network of trusted providers.
Care Coordination Services
Care Coordination services are provided by dedicated volunteers. Each volunteer is vetted through a comprehensive process including an interview, training and orientation, Live Scan and DMV background checks and reference checks. Volunteers provide a variety of high-quality services including:
- Grocery Shopping
- Friendly Visiting
- Check-In Calls
- Escorted Transportation
- Trusted Referrals
- Minor Home Repairs
- Home Safety Inspections
- Mail Sorting and Organization
- Pet Care
- Plus Much More!
What is Care Coordination?
Every member receiving Care Coordination services will have access to a Care Coordinator, a degreed social worker who is available to help you or your loved ones navigate care options. Care coordination services include:
- Care Transitions
- Family Caregiver Support
- Health Literacy
- Care Planning and Assessment
With this support, older adults remain independent and in their homes longer. Care Coordination lightens the load for caregivers and decreases the likelihood of older adults falling victim to scams and elder abuse.
ElderHelp’s programs are structured to help older adults maintain their independence. It is essential that you, as our member, are able to communicate directly with us, complete any assessments and interviews, and maintain a schedule. Currently, ElderHelp is unable to assist seniors with moderate to severe cognitive impairment. It is essential that the person needing help makes the initial call to begin services and is able to manage their own appointments and schedules. While it may seem easier to set up services for your loved one, the program will not be successful unless the individual who will be receiving services is interested, informed and engaged. ElderHelp does recognize the importance of a good support system and how valuable family is to the team.

Enrollment is easy! It just takes a call to ElderHelp to initiate a quick questionnaire over the phone. ElderHelp’s Enrollment Department will then schedule a home visit to confirm eligibility and help create your personalized package.

ElderHelp does not charge fees for services. Instead, we offer opportunities for our members to donate what they can for the services they receive.
