Care Managers are carefully trained, degreed social workers on staff at ElderHelp who are available to seniors and their families at no or low cost. Care Managers are geriatric professionals with experience working one-on-one with the elderly and connecting them to resources at ElderHelp and in the community.
 
Personal Care Coordination
Each Concierge Club member has their own Member Liaison and personal Member Care Manager who provide:
  • in-home assessments and screenings, including social, safety and legal needs
  • ongoing case management
  • problem solving
  • connection with vetted service providers
  • physician advocacy
  • assistance with completion of forms
  • support with family meetings and end-of-life issues
  • coordination of all home care services, including Concierge Club member benefits
  • and personalized matchmaking with caring volunteer caregivers.

Member Care Managers also advocate on behalf of low income members for cash assistance for home repairs, medical expenses, dental work, hearing aids, rental aid and more.
